The following information is supplied relative to your Group Life Insurance policy offered through Jersey City Lodge # 4. If it is your intention to maintain coverage the following chart shows what you will pay per year according to your age and benefit payable upon death.

For your information, this policy is a decreasing term policy. The face value decreases, as you get older as do the payments. The maximum value is for the younger members when the need is the greatest. Please make checks payable to JERSEY CITY FOP LODGE # 4.
